Job Details

LPN visits hospice patients in their home or in their skilled nursing facility.

Following each patient’s plan of care, the LPN provides direct care and documents each visit using the Netsmart EMR system; reporting any changes in the patient's condition to the RN Case Manager.

LPN can choose from (2) work schedules : 8am to 4 pm OR 4 pm to 12 midnight
