CMA / Care Coordinator

CSI Companies is looking for Medical Assistants to assist in quality care of those with nephrology. This is a direct hire position on a Hybrid schedule. Title : Medical Assistant Pay $17 / hr to $22 / hr DOE Hours : Full Time on a hybrid schedule Location : Great Philadelphia, PA

Responsibilities :

• Creates, reviews, and revises patients plan of care.
• Documenting patient labs, blood pressure, blood sugar, weight, diet / nutrition, and any other health behavior than could affect outcome
• Establish patient goals and interventions
• Ensures stage appropriate referrals for education, vascular access, and transplants are completed
• Maintains consistent communication with all patients
• Participates in patient enrollment in programs by gathering all clinical elements necessary.
• Educate patients about benefits of the programs.
• Performs medication reconciliations
• Collaborates with several specialists on necessary care and services
• Acts as a patient advocate, liaison, and information resource
• Monitors and reports patient outcomes through plan of care
• Provides chronic care education for nephrology and chronic / complex conditions
• Provides culturally appropriate education
• Performs timely and accurate documentation
• Performs other related duties as assigned or needed.
