Position Highlights :

Part-Time Position

Previous experience in a Skilled Nursing Facility (SNF) strongly preferred

Candidates must hold a BSW or MSW

Key Responsibilities :

Provide emotional and psychosocial support to residents and families

Assist with discharge planning and care coordination

Ensure compliance with federal and state social work regulations

Advocate for resident needs and quality of life
