Join Our Team as a part-time Pharmacist at Vital Care of Crown Point! Are you a dedicated pharmacist looking to make a difference in the lives of patients? Vital Care of Crown Point is seeking a part-time Pharmacist to join our team at our licensed home infusion pharmacy with a USP 797 compliant sterile compounding facility. We provide specialty infusion medications, nutritional support, infusion pumps, and more to patients in their homes and in our ambulatory infusion suite. Supported by Vital Care Inc., with over 150 locations nationwide, we offer comprehensive infusion therapy management in a state-of-the-art setting. This position could transition into a full-time position based on business needs. Daily Responsibilities : Perform pharmacy activities in compliance with regulatory and ethical standards, demonstrating sound business knowledge. Oversee the physical maintenance and organization of the pharmacy. Uphold regulatory mandates on the provision of health care by professionals. Act as a resource for departments and employees in resolving questions, problems, and issues. Implement and uphold company policies, procedures, and protocols in providing service to consumers. Is involved in strategic planning for the company to increase revenue and continually improve the quality of care provided to consumers. Maintain knowledge of USP chapter

to assure the quality, sterility, and stability of compounded sterile preparations Perform chart and lab results review for acceptance, product selection, device selection and supply management. Collect, evaluate, and interpret the physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety, and completeness. Monitor patient’s progress and assessment and communicate clinical information and results to the physician and healthcare team. Participate in on call rotation as indicated by staffing and business needs. Required Skills : BS or PharmD and active pharmacist license in good standing with IN Board of Pharmacy. Good interactive skills with employees, referral sources, physicians, and consumers. Excellent written and verbal customer service skills and advanced computer skills. Business and financial understanding of operating a profitable business. Three (3) years clinical experience preferred.
