The LVN will primarily focus on monitoring patient symptoms through technology and telehealth visits. With excellent attention to detail, the LVN will monitor and document symptoms, health data, and current medical conditions. The successful LVN will be able to provide consistency of care with patients and work closely with the in-house clinicians to ensure positive outcomes. Additional responsibilities for the LVN will include :

• Collaborating with in-house clinical team on follow up plan with patients who are being monitored between visits using a wireless monitoring devices and mobile app.
• Documenting and reviewing patient charts with accurate diagnoses and current conditions and responses.
• Communicating regularly with patients on concerns, current conditions, and responses to treatments.
• Monitoring wireless device and app data for changes in responses and health outcomes.
• Escalating critical issues to clinicians on patient health.
• Documenting all interactions with patients in the company portal.
