Position : Case Manager 1

Program : Womens Rise

Location : New Haven

Salary : $ 18.50 / hourly

Schedule : Full time 40 hours 1st shift Tuesday Thursday Friday Saturday (7-3pm) & 2nd shift Wednesday (3-11p)

Program Summary :

Womens Rise is a residential work release program that provides structure supervision case management vocational skill development support a to adult women who are under the supervision of Parole. We help women rise as they reenter their communities.

Position Summary :

The Case Manager 1 is responsible for providing staff support and direct client care. Responsibilities include delivering comprehensive case management services to clients including intake assessments development and implementation of individual service plans and liaison with treatment and community resources. This position is non-exempt and works under the supervision of program Management.

The ideal candidate is able to communicate in a respectful and professional manner with clients and has a trauma informed approach. Regular and predictable attendance is required.

Working within Adult Services the Case Manager 1 works with Community Justice program staff to help to rehabilitate educate and successfully guide clients from incarceration back into the community.

Requirements :

High School Diploma
2 years of related experience in the direct service and case management field
Valid Connecticut drivers license
Regular and predictable attendance is required

Orientation :

If selected for employment with The Connection newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
