Description

Job Summary :

• Evaluate and admit patients to home care; develop plan of care and provide care to patient according to plan.
• Act as case manager making necessary adjustments in patient's plan of care related to symptom changes and changes in care needs.
• Review and plan patient care and or discharge during scheduled case conference meeting and document accordingly.
• Act as a liaison for patient / caregiver with MD and community healthcare partners.
• Strive to lower cost of care through managing mileage, supplies and visit scheduling.
• Completeall documentation related to patient contact, assessments, SOC, and recerts.
• Resumption of care and discharge according to policy.
• Maintain understanding of current home care regulatory guidelines.
• Provide on-call coverage for both home care and hospice as assigned.
• Demonstrate and encourage team behavior and exceptional patient / guest experiences.
• Uphold and promote patient safety and quality.

Qualifications

Education :

Associate's degree in nursing and currently licensed by the state of Alabama required.

Experience :

Minimal 2 years' experience in a hospital setting working in medsurg, critical care or ER. Previous home care or hospice experience preferred.

Additional Skills / Abilities :

Must exhibit strong critical thinking and organizational skills. Must be able to work independently and make independent decisions. Must be flexible in scheduling. Must be able to organize and prioritize a schedule to meet patient needs.
