Join Our Team as a Community Support Specialist!

At Circle of Care, St. Louis, we’re passionate about supporting individuals and families, and we’re seeking a dedicated Community Support Specialist (Hybrid) to join our team. This role combines office-based support with field-based caregiving and outreach.

What You'll Do

Conduct intake interviews, home visits, welcome visits, and case management visits in the community.

Support individuals transitioning from Skilled Nursing Facilities (SNFs).

Assist with community integration and benefits navigation.

Provide personal care support (ADLs, respite, APC tasks if authorized).

Recruit new clients and attend community events and resource fairs.

Complete accurate documentation in CareSmartz360 and Charity Tracker.

What We’re Looking For

At least 1 year of experience in social services, healthcare, or a related field.

Compassionate, organized, and an excellent communicator.

Able to work a flexible schedule (includes evenings / weekends).

Able to lift at least 35 lbs to assist clients.
