About the Role :

The Registered Nurse (RN) plays a critical role in delivering comprehensive patient care within home health and hospice settings, ensuring patients receive compassionate and skilled nursing services tailored to their individual needs. This position involves managing direct patient care, including wound management, telemetry monitoring, and patient education, to promote optimal health outcomes and quality of life. The RN serves as a patient advocate, coordinating with interdisciplinary teams to develop and implement personalized care plans that respect patient preferences and clinical requirements. Through continuous assessment and monitoring, the RN identifies changes in patient conditions and responds promptly to ensure safety and comfort. Minimum Qualifications :

• Current and valid Registered Nurse (RN) license in the state of practice.
• Minimum of 2 years of clinical nursing experience, preferably in home health or hospice care.
• Basic Life Support (BLS) certification.
• Demonstrated proficiency in direct patient care and nursing documentation.
• Strong communication and interpersonal skills.
