Job Description & Requirements Specialty : Physical Therapist Discipline : Therapy Duration : 13 weeks 40 hours per week Shift : 8 hours, days Employment Type : Travel Schedule : Monday-Friday Guaranteed Work Week : 40 Coverage : Nashua and surrounding May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address. Average 4-6 visits per day depending on type of visit for PT / OT / SLP Productivity is 28-32 points per week for PT / OT / SLP SOC : 2.5 points, ROC : 2 points, Evaluations / Recert / Discharge OASIS : 1.5 points, Discipline DC, routine visit, 30-day reassessment : 1 point, Weekly case conference : 1 point Any overtime must be pre-approved. Home Health and Oasis experience required, no exceptions Homecare Homebase documentation experience required Client may consider without HCHB exp if the manager is willing to train.
