$7,500 Sign-on Bonus - Pinellas Country - Reporting to the Clinical Care Manager, the Triage RN is responsible for identifying, prioritizing, and coordinating patient / family needs for the purpose of providing preventative and supportive care to the terminally ill patient / family unit with the collaboration of the interdisciplinary team.
This position is primarily performed at employee’s home. Work environment may be stressful at times, as overall activities and work levels fluctuate.
Shift : Monday - Friday 5pm - 8a
EDUCATION AND QUALIFICATIONS :
- Currently licensed in the State of Florida as a registered nurse
- Minimum of two (2) years nursing experience
- Hospice, oncology, or home health nursing experience preferable
- Minimum of one (1) year experience in an acute care unit is desirable
- Minimum of one (1) year experience in caring for terminally ill patients is desirable
- Should have training in palliative care and symptom management
ESSENTIAL JOB RESPONSIBILITIES :
Attends to patient / family complaints and concernsTakes a proactive approach to identifying and resolving potential issuesEducates the community regarding hospice care, philosophy, and concept through individual consultation with persons in the communityPerforms other duties as requested by the Clinical Care Manager.Provides assessment of the physical, psychological, social, and spiritual needs of the patient / family unit and initiates appropriate interventionDetermines the health status of the patient based on hospice criteria and identifies patient / family issuesProvides follow-up for information related to patient care issues and continuity of care to the appropriate CCM / RN Case ManagerReceives and returns telephone calls during non-business hoursAccesses and supplements patient data via current computer programCommunicates pertinent information to the CCM and necessary disciplines to ensure continuity of careCoordinates and identifies services needed by patient / family unit after hours regarding continuous care, transfers, staffing, physician’s orders, referrals, pharmacy, and durable medical equipmentDemonstrates knowledge of Gulfside Hospice policies and procedures and pertinent federal and state regulationsConsiders financial implications in rendering cost-effective nursing careEnsures professional standards of nursing practice are maintainedPerforms other duties as requested by the Clinical Care Manager.
