Primary Care Nurse Practitioner
We are seeking a dedicated and skilled Nurse Practitioner to join our team at a leading healthcare facility in Modesto, CA. In this role, you will work directly under the guidance of the Medical Director and collaboratively with Physicians and the interdisciplinary team (IDT) to provide top-notch primary care and medical assessments for our senior participants. Salary : $160,000.00 - $180,000.00 per year
Essential Duties and Responsibilities :
- Conduct in-person comprehensive assessments including initial, semi-annual, unscheduled assessments, and continuous medical coverage.
- Develop individual care plans in collaboration with physicians and the IDT for each participant.
- Assist in discharge planning from acute or long-term settings in collaboration with the medical team.
- Regularly visit participants in nursing homes based on their needs to ensure continued care.
- Review outcomes and utilization data under the direction of the Medical Director to establish best practices and policy improvements.
- Participate in rotating night and weekend calls to ensure 24 / 7 care availability.
Minimum Position Qualifications :
Graduation from an Accredited School of Nursing.Certificate as a Nurse Practitioner with Prescriptive Authority.Active RN State License in California.Minimum of two years of experience working with the senior population.Current knowledge of chronic care / geriatric issues and best practices.Valid State Driver's License with a clean DMV record.Proof of recent Vehicle Insurance.Preferred Qualifications :
Experience in a PACE program setting or similar geriatric care facility.Strong communication and interpersonal skills to effectively manage care coordination.Ability to respond effectively under high pressure or emergency situations.Skills :
Proficient in creating and following care plans tailored to individual needs.Strong collaboration skills to work effectively with physicians and an interdisciplinary team.Excellent assessment and diagnostic skills relevant to geriatric medicine.Ability to manage multiple cases simultaneously while maintaining attention to detail.Benefits :
