Description

We are seeking a dedicated and compassionate Community Health Worker (CHW) to join our team and provide essential support to telephonic nurses in managing high-risk Medi-Cal patients. The CHW will serve as a vital link between patients, healthcare providers, and community resources, working to improve patient health outcomes and reduce health disparities. This role requires a proactive approach to in-person patient engagement and the ability to work effectively in diverse settings. The CHW will play a pivotal role in locating hard-to-reach members out in the community, including FQHCs, hospitals, food pantries, shelters, and the surrounding community.

This position will primarily take place out in the field in the Lancaster / Palmdale area, with some tasks to be completed at home.

What You'll Do

Conduct outreach to high-risk MediCal patients in the IE or Antelope Valley, as identified by telephonic nurses.

Perform assessments of patients after completing in-person engagement.

Connect patients with community resources such as food assistance, housing support, transportation, and social services.

Maintain accurate and timely records of patient interactions and activities.
