Rn,case manager

We are seeking a dedicated and skilled Hospice Registered Nurse Case Manager to provide care to hospice patients to join our healthcare team. The ideal candidate will play a critical role in coordinating patient care, ensuring quality outcomes, and facilitating communication among healthcare providers.

Hybrid-based from home and traveling in the field in the

Park Ridge / Skokie / Chicago IL area

Responsibilities

Completing an initial assessment of patient and family to determine hospice care needs

Regularly re-evaluates the patients nursing needs

Use assessment data to determine care plan needs

Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated

Counsels the patient and family in meeting end of life needs

Initiates appropriate preventative and palliative nursing procedures

Communicates with the physician regarding patient's needs and reports any changes in the patient's condition

Providing pain management and comfort for patients with terminal or chronic illnesses.

Oversees the hospice services for these patients, working with the patient's families and caregivers to determine the best care possible.

Services may be provided in the patient's home, at the hospital, or in a specialized hospice facility.

Your job duties include meeting with the patient and their families to discuss the care plan, confirming insurance coverage and other payment options, and monitoring the condition of the patient and making adjustments in care as necessary.

Requirements :

Homecare and hospice background preferred, will consider transferrable skills

Must have active IL RN licensure

Active and clear Illinois Driver's license and reliable vehicle

Schedule : M-F 9am-5pm
