Reason for Coverage : Provider FMLA Practice Setting : Hospital coverage with ambulatory follow-up as needed

In house while performing surgery and rounding. Generally, 8 hours per day. Unrestricted call for time thereafter. Schedule : Two 24 on call hour shifts, with call back hours, and the third day 8 hours on site for cleanup from the two on call shifts.

All trauma presentations to include pelvis, acetabular, and supracondylar humerus fractures. Pelvis and acetabular are required procedures. Estimated Credentialing Timeframe : 45-60 days
