Job Description

Looking for a compassionate and skilled Licensed Practical Nurse (LPN / LVN) to join our team. You'll work under the supervision of a Registered Nurse to provide direct patient care in the patient's home.

Your responsibilities will include :

• Delivering hands-on nursing care to a diverse group of patients.
• Assisting the RN in assessing patient needs (physical, psychosocial, and environmental).
• Participating in the planning and implementation of patient care plans .
• Communicating effectively with patients, families, and the interdisciplinary team.
• Ensuring all care aligns with infection control and safety standards.
• Maintaining accurate and timely documentation to ensure continuity of care .
• Promoting the philosophy and policies of our hospice / home health program.

Qualifications

Transportation : Reliable transportation. Valid and current auto liability insurance.
