Social Worker

Imagine360 is seeking a Social Worker to join the team!   The Social Worker provides resource coordination services and support to program participants with medical, behavioral, and social needs. Working collaboratively with a multidisciplinary care team, this role supports case management and disease management efforts by assessing social determinants of health (SDoH), facilitating care transitions, and promoting health outcomes through barrier reduction, patient engagement, and self-management strategies.

Position Location :   100% remote

Responsibilities include but are not limited to :

• Conduct needs assessments based on direct referrals from our Care Management team or case reviews.
• Collaborate with Case Managers and Care Coaches, and other care team members to develop and implement individualized care plans.
• Identify and address barriers to care including housing instability, transportation, food insecurity, mental health, substance use, and financial concerns.
• Support program participants in navigating healthcare systems and community resources to promote continuity of care.
• Monitor program participants progress and adjust interventions in collaboration with the Care Management team to achieve desired health outcomes.
• Document all assessments, care plans, interventions, and outcomes in accordance with organizational policies and regulatory requirements.
• Maintain up-to-date knowledge of community resources, benefits, and entitlement programs.
• Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives.
