The Heart of the Role : Your Impact
As the Resident Care Coordinator, you are the link between clinical needs, family communication, and regulatory compliance.
Care Plan Oversight : Develop, implement, and monitor comprehensive, individualized care plans for residents, ensuring they are accurate, current, and followed by all staff.
Clinical Quality : Monitor and evaluate the effectiveness of nursing care and clinical programs, leading quality improvement initiatives to enhance resident outcomes.
Regulatory Compliance : Ensure all resident care documentation, assessments (e.g., MDS, care plans), and processes meet federal, state, and facility standards. This isn't just paperwork; it’s proof that we're doing things right.
Interdisciplinary Team Lead : Facilitate communication and coordination among nurses, physicians, therapists, families, and outside healthcare providers.
Family Liaison : Serve as the primary clinical point of contact for residents and their families, addressing concerns and providing updates with empathy and clarity.
What We Need You to Bring (The Essentials)
Active Registered Nurse (RN) license in the state of Pennsylvania (Mandatory).
Minimum of 2-3 years of clinical experience in a long-term care, skilled nursing, or assisted living environment.
Proven experience in developing and managing resident care plans, preferably with knowledge of the Minimum Data Set (MDS) process.
Exceptional organizational skills and the ability to manage multiple priorities in a fast-paced setting.
Strong verbal and written communication skills—you'll be talking to doctors, CNAs, and concerned family members all in one day.
