Job Title : Registered Nurse I

Experience : Minimum 1 year of adult home health or hospice experience

License : Must have license in hand

Certifications : CPR, BLS

Must-Have : Proficiency in OASIS and HomeCare HomeBase (HCHB) EMR

Job Description :

• Provide, coordinate, and direct the provision of home nursing care according to physicians' orders and agency policies and procedures.
• Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
• Conduct home visits and attend periodic office meetings; reliable transportation, a valid driver's license, and liability insurance are required.
• Manage an average of 5-8 patients each day, translating to roughly 6-8 productivity points.
• Perform various types of cases including Start of Care, Wound Care, and Follow-up Visits.
• Documentation must be completed within 24 hours of patient appointments.
• Coverage includes a territory requiring travel up to 75 miles from the home clinic.
• Compensation for travel will be provided based on mileage between patient visits; commute to and from the first and last patient is not compensated.
• Must be open to on-call rotations and floating to nearby clinics if patient census is low.
• Must maintain proficiency in HomeCare HomeBase (HCHB) and have used the system within the last six months, or undergo a retraining course.
• Dress code requires wearing scrubs.

Please note that the position involves driving to patients' homes daily, and attending weekly clinic meetings. Candidates must accept all patient assignments and complete timely documentation to ensure seamless billing and follow-up care.
