Community Living Center Nurse Assistant
The Community Living Center is a mix of short stay rehabilitation, long term and palliative care residents. He / she is expected to work closely with the licensed staff and interdisciplinary team to deliver comprehensive, efficient, coordinated care to residents with complex medical conditions. He / she approaches each resident's care from a holistic perspective focusing on the resident's physical, psychological, social, and spiritual well-being.
Responsibilities include :
- Utilizing glucose monitoring machine to receive glucose readings for veterans and referring deviations to appropriate health care staff.
- Providing direct care for complex veterans.
- Making observations of subtle changes in resident's condition and taking action accordingly.
- Documenting care given, observations, and responses on appropriate tools.
- Performing nursing procedures within the scope of practice.
- Working on small group activities with veterans / residents.
- Recognizing residents' autonomy and promoting independence.
- Participating in treatment planning meetings.
- Utilizing equipment to promote veteran self-care.
- Maintaining a clean, safe environment for the patient / resident.
- Serving as the unit safe patient handling liaison.
- Acting as a preceptor and mentor to other NAs.
- Managing distributed patient behavior and intervening appropriately.

This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity.This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing.Transferring patients and objects may be required.The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing.The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.Additional Information :
