Job Description

Job Title : Registered Nurse

Shift : Day 5x8-Hour (08 : 00 - 16 : 30)

Guaranteed Hours : Facility can call off up to one shift per pay period (every two weeks)

Experience : Minimum of 1 year of skilled home health experience required

License : Pending license accepted

Certifications : BCLS Required

Must-Have : COVID vaccination primary series and one booster

Job Description :

• Provide skilled nursing care to patients in home health settings.
• Visit up to 6 patients per day, adhering to a 1 : 1 ratio.
• Professional attire required; scrubs of any color are acceptable.
• Proficiency in EPIC charting system.
• Complete a standard of 30 productivity points daily.
• Use reliable personal transportation for patient visits, with mileage reimbursed at the IRS standard.
• Complete a 40-hour orientation (non-billable).
• Attend to hospice care needs within a 50+ mile occasional maximum travel radius.
• Participate in modules pre-start and annually, with an average of 16 hours spent.
• Expected to have supervisory references covering a minimum of 1 year within the last three years in a comparable specialty / setting.
• First-timers are accepted and encouraged to apply.
• Warm and engaging disposition with the ability to work independently.
• Facility or agency will provide necessary communication devices, or reimburse work-related phone expenses.
