Your Key Responsibilities :

• Participate in interdisciplinary team meetings and conduct home / clinic assessments as required.
• Perform initial and periodic in-person assessments to evaluate patients' functional abilities and rehabilitation needs.
• Recommend and coordinate adaptive equipment and home modifications to enhance safety and independence.
• Develop, implement, and update individualized treatment plans based on patient progress and team collaboration.
• Deliver physical therapy interventions in clinics, patients' homes, and affiliated facilities.
• Educate patients and their caregivers on equipment usage, home safety strategies, and activities of daily living programs.
• Document assessments, treatments, and progress in a timely and accurate manner, adhering to regulatory and organizational standards.
• Engage actively in daily interdisciplinary team meetings, contributing to comprehensive care planning and coordination.
• Supervise Physical Therapy Assistants (PTAs), review care plans, co-sign documentation, and provide mentorship.
• Train staff on safe PT practices and equipment use to enhance participant care.
• Support quality improvement and compliance initiatives.
• Perform additional duties within scope as needed.
• Maintain relevant licensure, certifications, and pursue ongoing professional development.
• Ensure confidentiality and proper access to Personal Health Information (PHI) based on organizational guidelines.
