PRN Physical Therapist

Your Key Responsibilities :
- Participate in interdisciplinary team meetings and conduct home / clinic assessments as required.
- Perform initial and periodic in-person assessments to evaluate patients' functional abilities and rehabilitation needs.
- Recommend and coordinate adaptive equipment and home modifications to enhance safety and independence.
- Develop, implement, and update individualized treatment plans based on patient progress and team collaboration.
- Deliver physical therapy interventions in clinics, patients' homes, and affiliated facilities.
- Educate patients and their caregivers on equipment usage, home safety strategies, and activities of daily living programs.
- Document assessments, treatments, and progress in a timely and accurate manner, adhering to regulatory and organizational standards.
- Engage actively in daily interdisciplinary team meetings, contributing to comprehensive care planning and coordination.
- Supervise Physical Therapy Assistants (PTAs), review care plans, co-sign documentation, and provide mentorship.
- Train staff on safe PT practices and equipment use to enhance participant care.
- Support quality improvement and compliance initiatives.
- Perform additional duties within scope as needed.
- Maintain relevant licensure, certifications, and pursue ongoing professional development.
- Ensure confidentiality and proper access to Personal Health Information (PHI) based on organizational guidelines.

Graduate from an accredited Physical Therapy program meeting American Physical Therapy Association standards.Valid South Carolina Physical Therapist license required, along with successful completion of national examination.Preferred 3 years of diverse PT experience.Experience with geriatric / frail populations is a plus.Strong communication skills for effective collaboration with clinical and non-clinical staff.Ability to multitask and manage responsibilities in a dynamic healthcare setting.Competence in interpreting technical patient care plans and procedures.Proficient in assessing patients using visual and auditory cues.Adaptable and resilient, responding effectively to changing patient needs.Must possess a valid driver's license and auto insurance.Current CPR / BLS certification required.Ability to pass drug screening and background checks.Up-to-date immunizations (TDP, MMR, Varicella, Flu, Pneumonia) and a health statement.Meet all health and physical requirements to provide patient care safely.Why Choose Interim HealthCare?
