Psychiatric Rehabilitation Services Director

Skilled nursing facility is seeking a  Psychiatric Rehabilitation Services Director (PRSD) to whom the resident primarily relates for the coordination of services, programs, oversight of data collection and review of resident performance. LTC experience is needed for this postion. The candidate must meets the following qualifications :

• Masters degree in human services
• At least 1 year of Experience with residents, crisis intervention, documentation, care planning with involving families in the process, knowledge of the MDS, d / c planning, facilitating skills groups and psycho-social groups and problem solving.Must be able to work in an interdisciplinary team environment.Must be able to handle confidential data with professional discretion
