Monday - Friday 8 : 30am - 5 : 00pm, also working every 5th weekend answering phones after hours.

Job Duties :

  • Assumes responsibility for making and / or receiving phone calls. As well as responsibility for maintaining hospitalization list, including notification of all pertinent staff.
  • Monitors referrals coming into Agency via fax or hand delivered by a member of the marketing team.
  • Responsible for maintaining the referral log in an accurate and timely manner, including patient NTUC (not taken under care).
  • Accurately enters referral information verifying demographics and physicians. Collects all face-to-face documentation for the CRS to review.
  • Initiates referral documentation in EMR software from MD offices, discharge planners, hospitals, and home care agencies.
  • Applies effective, respectful and positive verbal and written communication methods in the daily work setting to promote a collaborative work environment.

    Who you are?

  • High School graduate; College degree preferred.
  • Microsoft Office proficient.
  • Exceptional organizational and multi-tasking skills.
  • Time management skills required.
  • Experience with Kinnser / Focura and Allscripts is a plus.
