Envision is a local non-profit agency who has been supporting individuals with disabilities for over 60 years! Our Supported Living program provides a variety of customized support services that make it possible for an adolescent or an adult with a developmental disability to live safely and comfortably in their own home or in a group living arrangement.

Our Care Coordinators develop meaningful and person-centered relationships with the individuals served, their family members, and team of staff. They create customized supports for each individual that embraces positive behavior supports and engagement in a full and meaningful life. The supervisory role of the Care Coordinator provides oversight and coordination of all services for the individual’s care and day to day needs; develops a customized, engaging milieu for individuals including group and individualized activities; supervises and trains direct support professionals in the care of individuals receiving services; provide support to adults receiving services in the development of : coping skills, social skills, communication skills, activities of daily living skills, and expansion of leisure interests and skills; coordinates care and support schedule, ensuring coverage of all scheduled times of day with respect to the approved service plan; provides high level collaboration and communication with all service providers and family members.

Care Coordinators must be able to switch between tasks, supervise others, and lead by example, with dedication, positive attitude and support to develop a good working rapport with the individual, family members and other support team members. The Care Coordinator reviews all service documentation for compliance with all applicable rules, regulations and standards.

MINIMUM JOB REQUIREMENTS

Position Requirements (Required minimum qualifications) :

  • Bachelor’s Degree in Psychology, Social Work, Special Education, or a related human services field OR 2+ years of relevant experience in the I / DD demonstrating increased responsibility and understanding
  • An insured, reliable vehicle for possible on-the-job use
  • Driver’s license with no more than 5 points

Required Experience :

  • At least 2 years in the field working with individuals with developmental disabilities, mental health diagnosis or co-occurring developmental disabilities and mental health diagnoses
  • At least 1-year supervisory experience
  • Preferred Experience :

  • Experience with Trauma Responsive Model of Care
  • Experience with E-MARS or other similar electronic documentation systems
  • Experience supporting individuals in hospital, mental health treatment or school settings
