In this role, you will coordinate long-term care needs for patients within their communities, working in the field and ensuring the best possible outcomes. You will have the support of an elite and dynamic team at your side, all while leveraging your diverse knowledge and experience to enhance healthcare services.

As a Health and Social Services Coordinator, you will play an essential role in our Integrated Care Model, collecting key information regarding member needs, advocating for optimal care, and ensuring access to the right services.

When located in Oahu, HI, you will have the flexibility to work remotely while addressing challenging issues. Expect to spend about 75% of your time visiting our members in their homes or long-term care facilities. Our teams are based in the downtown Honolulu area as well as the West Side, East Side, and North Shore regions. Flexibility, adaptability, and patience are paramount in this role.

Key Responsibilities :

• Assess, plan, and implement individualized care strategies aimed at the most appropriate, least restrictive level of care.
• Identify and refer patients to social service programs, addressing financial, psychosocial, community, and state support services.
• Manage care plans throughout the continuum, serving as a single point of contact.
• Communicate necessary health-related information to all stakeholders to ensure that quality, coordinated care and services are provided effectively.
• Advocate for patients and families, ensuring their needs and choices are represented and supported by the healthcare team.

Pay is based on various factors, including local labor markets, education, work experience, and certifications.
