A Licensed Health Aide (LHA) is a paraprofessional who is specifically trained, competent and performs assigned functions of personal and skilled care to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN). In this role, you will report to the Director of Nursing (DON) / Nursing Supervisor / RN.Duties / Responsibilities : Provides services that are ordered in the plan of care and that the aide is permitted to perform under State lawPerforms hands-on personal carePerforms simple procedures as an extension of the therapy or nursing services as assignedAssistance in ambulation or exercises as assignedAssists with medications that are ordinarily self-administered as assignedUnderstands and adheres to established Agency policies and proceduresCompletes appropriate visit records in a timely manner as per Agency policyReports changes in the patient's condition and needs to the appropriate personnelPerforms household services essential to health care in the home as assignedAttends staff meetings and patient care conferences as scheduledFollows Agency policy for cleaning equipment between patient usesCarries out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviewsCompletes all work assigned by the RNDoes not accept assignments for a patient with special needs for which he / she has not received appropriate trainingObserves confidentiality and safeguards all patient related informationReports any variance, accident or unusual occurrence to the appropriate personnelParticipates in QM activities as requestedMaintains acceptable attendance status, per Agency policyReports incomplete work assignments to RNAppearance is always within Agency standard; is clean and well groomedDemonstrates effective time management skills through daily documentation and infrequent overtime for routine assignmentsAttends position related in-services.
