Job Title : Physical Therapist - Skilled Nursing and Long-Term Care Facility Full Time

Location : Bellevue Health and Rehabilitation 6500 N Portland, Oklahoma City, OK. 73116

Summary : This position is responsible for providing comprehensive physical therapy services to residents of our skilled nursing and long-term care facility, focusing on restoring and maintaining their function, mobility, and independence.

Conduct thorough evaluations of residents to determine functional limitations, develop individualized treatment plans, and set measurable goals.Review medical history, diagnoses, and referrals from physicians and other healthcare professionals.Observe residents during functional activities to assess movement, range of motion, strength, and balance.Develop individualized treatment plans that address the resident's specific goals, needs, and expected outcomes.Collaborate with physicians and other healthcare team members (nurses, occupational therapists, etc.) to ensure coordinated care.Set measurable goals and objectives for each resident.Implement prescribed treatment plans utilizing various physical therapy techniques, including exercise, modalities, equipment, assistive devices, and patient education.

Requirements :

Education, Experience, and Licensure Requirements :

Graduate of an accredited program in physical therapy, BSPT, MSPT, or DPT (2003 or later must have MSPT or DPT)Currently registered / licensed in applicable State. Must maintain an active license in good standing throughout employment.CPR / BLS certification upon hire or obtain during orientation. CPR / BLS certification must remain current during employment.

Interdisciplinary Collaboration :

Communicate effectively with residents, families, and other healthcare team members.Participate in interdisciplinary team meetings to discuss patient care plans and progress.Oversee and evaluate care given by PTAs, Rehab Aides, and PT / PTA students
