Salary : $28 - $32 / hourly (depending on experience)

JOB SUMMARY :

Friends of Children with Special Needs (FCSN) is currently seeking a dedicated and compassionate CFS (Coordinated Family Support) Program Coordinator to join our team. The CFS program is designed to support adults with developmental disabilities who choose to live with their families. It is a personalized, flexible, and individualized service that adapts to the evolving needs and preferences of both the individual and their family. The CFS Program Coordinator plays a key role in assisting individuals and their families in achieving their goals through the CFS services. This role involves collaborating closely with individuals, their families, Regional Centers, and other service providers to ensure the delivery of high-quality, person-centered services. The ideal candidate will possess strong communication skills and a genuine passion for making a positive impact in the lives of others.

RESPONSIBILITIES :

  • Meet with the individuals and their families to assess their needs on regular basis, and to

develop and implement personalized support plans that reflect each individuals needs,

preferences, and goals.

  • Communicate in the individual and family preferred language and must be respectful of
  • their culture.

  • Coordinate and monitor the delivery of services, ensuring the highest standards of care
  • and compliance with DDS and RC requirements.

  • Communicate effectively and proactively with the Program Director and Program
  • Manager.

  • Respond proactively to questions, concerns, and issues raised by Regional Centers,
  • parents, and others service providers within program confidentiality guidelines.

  • Document the individual training and support services using data collection and
  • participant log forms, and prepare reports documenting participants progress toward ISP

    goals & objectives;

  • Submit Special Incident Report to parents / legal guardians, Regional Centers and / or APS
  • in a timely manner

  • Attend community outreach events to promote CFS
  • Utilize Person-Centered Thinking (PCT) strategies, including ISP meetings and semi-
  • annual reviews.

    REQUIREMENTS :

  • Education : An Associate degree in human services field.
  • Experience : Preferred 3+ years of experience in developmental disabilities services, with a focus on direct support.
  • Skills :
  • Knowledge of developmental disabilities, DDS, Regional Centers systems, and Social Services.
  • Good communication abilities in oral and written, including working with diverse communities.
  • Mandarin is a plus
  • Proficient in Microsoft Office applications, and other relevant software.
  • Physical Requirements :
  • Ability to perform tasks involving lifting (up to 25 lbs), bending, and assisting with personal care.
  • Must have a reliable vehicle to travel for client-related needs and transport clients as needed.

  • Strong organizational, time management, and problem-solving abilities.
  • COMPANY-WIDE JOB REQUIREMENTS :

  • Pass DOJ fingerprint-based background clearance and health screening.
  • Possess a valid drivers license with clean driving record.
  • Obtain CPR and First Aid certification within one month of employment
  • Adhere to health and safety protocols as required by local authorities.
