• Work with a Joint Commission accredited agency What You’ll Do :
• Evaluate patients, develop personalized care plans, and provide skilled PT treatments
• Collaborate with physicians & the Director of Nursing
• Teach patients, families, and caregivers safe mobility & exercise techniques
• Monitor progress, document outcomes, and support discharge planning What We Require :
• Master’s degree (or higher) in Physical Therapy from an accredited program
• Active NJ PT license + CPR / BLS certification
• Minimum 1 year of PT experience (home health preferred)
