Certifications : Basic Life Support (BLS)
Must-Have : Critical care background
Description :
The Clinical Nurse is a Registered Professional Nurse who provides safe, competent, quality care based on nursing theory and research to a designated group of patients and significant others.
Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care.
Applies the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards.
Assesses and evaluates patient care needs and applies critical thinking skills in patient care management.
Integrates relevant assessment and intervention skills in the delivery of nursing care.
Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care.
Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease.
Communicates effectively and professionally with patients, families, and all members of the Interdisciplinary Patient Care Team.
Manages assignments within the Care Delivery Model and demonstrates effective allocation of resources.
Role models the principles of Relationship Centered Care through various patient care actions.
Considers patients' values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care.
Establishes and maintains a therapeutic relationship with patients and families.
Uses evidence-based practices to enhance understanding of patients' perceptions of care.
Initiates service recovery for patients and families who have concerns.
Demonstrates caring, respect, compassion, empathy, and active listening in all interactions.
Supports staff in using evidence-based practices to understand patients' perceptions of care.
Conducts purposeful rounding on assigned patients addressing essential care needs.
Practices safe and efficient patient-centered care, identifying barriers and opportunities for improvement.
Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served.
Implements process improvement strategies based on institutional standards and procedures.
Engages in formal and informal peer and institutional review processes.
Collaborates with interdisciplinary teams to create and implement quality improvement projects and research studies.
Contributes to quality and safety practices and required compliance measures.
Implements principles of high reliability to avoid high-risk behaviors and ensure patient safety.
Escalates potential safety hazards or gaps from best practice following the appropriate protocols.
Utilizes institutional information technology by documenting nursing practice to support improvement initiatives.
Performs charge nurse duties as assigned.
Coordinates with leadership to ensure supplies are utilized appropriately.
Escalates resource deficits to the appropriate management personnel.
Demonstrates patient-focused and cost-effective approaches to patient care in all areas.
Documents accurately in compliance with regulatory and legal standards of care.
Facilitates patient throughput on a shift basis.
Contributes to the environment of care to support colleagues, patients, and families.
Advances clinical competence to progress in nursing practice.
Projects a professional image and communicates with respect and empathy.
Incorporates ethical principles into decision-making for patient care.
