Field Care Manager – Behavioral Health (Social Worker)

Location : Wayne or Macomb Counties, MI

Travel : 15–20%, WFH the rest

Residency Requirement : Must reside in Michigan and be able to travel locally

Overview : Judge Healthcare is hiring a Field Care Manager to support Medicare / Medicaid members through in-home and community-based care coordination. This role is ideal for licensed social workers committed to improving health outcomes and promoting member independence.

Key Responsibilities :

• Manage a caseload of Medicare / Medicaid members through in-home, community-based, and telephonic assessments
• Develop and update individualized care plans in collaboration with members, caregivers, and providers
• Provide clinical guidance for members with complex medical and behavioral health needs
• Coordinate services across healthcare, housing, education, and social domains
• Conduct medical attestations and clinical oversight reviews
• Ensure care is delivered in the least restrictive setting
• Lead coordination efforts with the Interdisciplinary Care Team (ICT)
