A company is looking for a Health Coach (Remote). Key Responsibilities Conduct assessments of members' health status to improve medication adherence and health outcomes Utilize motivational interviewing to engage members and promote behavior changes for optimal health Develop proactive strategies to address health barriers and enhance overall wellness through program integration Required Qualifications 1-3 years of experience with Microsoft Office 3-5 years of practice experience in areas such as Renal, Diabetes, Hypertension, or Primary Care Experience in Managed Care and Medicare Must reside in the EST / CST time zone Hardwired internet connection is required
