LPN / Case Manager

Fountain Court Senior Living, located in Maple Valley, Washington is currently seeking an LPN to join our team.

LPN Duties and Responsibilities :

Maintains resident assessments per State regulations

Provides nursing care to residents as assigned

Administers and records medications and treatments as assigned

Supports and provides health care instructions to staff, residents and their families

Participates with other team members in planning, implementing and evaluating nursing care and other patient activities

Assists in the admission, transfer, and discharge of residents

Observes and reports residents' vital signs, symptoms and general condition to the appropriate individual(s)

Records on patient charts observation of patient’s responses to nursing care rendered

Maintains environmental conditions conducive to the comfort and safety of residents

LPN Qualifications and Skills :

LPN Experience :   2 years (Preferred)

WA State LPN License in good standing (Required)

Long Term Care or Assisted Living experience a plus

Salary negotiable based on proven experience
