Community Health Care Barberton is looking for a Family or Adult Nurse Practitioner with a passion for helping others to work three days a week. Responsibilities :

• Performing physical exams and patient observations
• Recording patients' medical histories and symptoms
• Creating patient care plans and contributing to existing ones
• Ordering and analyzing diagnostic tests
• Diagnosing health issues
• Detecting changes in patients' health and modifying treatment plans, as needed
• Educating patients and their families to manage and prevent illness and injury
• Collaborates with physicians in managing acute and long term medical needs of the patients
