This facility is seeking a Primary Care Home Health Physician or Nurse Practitioner (NP) for locum tenens support as they look to fill a current need.

Details and requirements for this opportunity :

• Schedule : Flexible scheduling based on availability (Part-Time or Full-Time basis)
• Patients per day : 6-10
• Practice Setting : In home health and wellness evaluations
• Scope : Home Care
• State Licensure : Connecticut
• Credentialing : 6-8 weeks
