Summary
Coordinate Case management services to the child(ren) and mothers in placement and manage the overall placement toward success
Primary Performance Objectives
- Incorporate CARE principles into daily living practice as Case Manager
- Document case management services and the progress of the child and family through maintaining up-to-date files on each client in the program, completing monthly and discharge reports on each client, care plans, assessments, and completing all other assigned paperwork
- Encourages and promotes placement of families through contact with public agencies, organizations, groups and private individuals
- Builds and maintains relationships with other placing agencies
- Adheres to Partnership in Achieving Independence through Recovery and Self Sufficiency Strategies (PAIRS) contract and submits all intake documents required for approval through PAIRS
Job Task Summary
Duties and ResponsibilitiesUnderstands and follows personnel policies and practices as outlined in the ECH Staff Manual and in compliance with the South Carolina Department of Social Services (SCDSS) group home regulations, and the C.A.R.E. practice modelCoordinates and participates in interviews for potential clientsPrepares the paperwork and the file for new admissions and maintains the historic file throughout placementAssist admission department with obtaining signatures on admission paperworkInputs intake and discharge data into the electronic database system and updates information as neededCoordinate services to children and families through referrals to campus and community-based service providersTo cooperate with all Epworth staff in order to enhance the mission and ministry as a whole. This would include role modeling and advocating on behalf of familiesAttend individual and group supervision meetings and trainings as scheduledRespond to behavior crises in the school or in the cottage whenever neededAssist in supervision of children and support the cottage team as neededCommunicate regularly with the community service providers to ensure that the providers are aware of the family's progress and of any other issues that arise with the caseWork in conjunction with the academic department to register and withdraw students from school, to communicate with public school teachers and staff, and to attend IEP meetings and conferencesAccompany clients on psychiatric appointments and follow all documentation procedures related to this processFunction as a member of an interdisciplinary team cooperating with and involving others to obtain common goals for each client (i.e. Intake, Health, Education, Activities, Religious, etc)Meet at least weekly with the cottage team to develop and implement plans for direct work with familiesParticipate as appropriate in the PQI process as assignedFollow direction and guidance of immediate supervisor. Additional duties may be assigned as neededQualifications
Experience working with children and families to provide case management services to children and mothers in placement at the Family Care Center.Master's Degree in a Human Service Field, preferred; bachelor's degree required.Knowledge of working with clients in substance use treatment preferred.Personal Attributes and SkillsDemonstrates the ability to learn and to practice teamwork; patient and compassionateUnderstands and respects the necessity for confidentialityPositive and calm presence in all situations.
