Responsible for multiple locations in Elgin, Geneva, St Charles and West Chicago.
What you will be doing?
The Manager of Community Health Services is responsible to coordinate community health services for individuals in I / DD Service Programs for assigned regions according to DHS, HCBS, and CARF requirements. The essential job responsibilities include collaborating with internal and external health care providers in the implementation of individualized strategies that mitigate identified health risks and maximize the client’s progress toward selected outcomes and community inclusion.
The Manager is responsible for the hiring, training, ongoing supervision, and performance evaluation of the Community Health Services team. This program support team ensures timely scheduling, completion, and documentation of routine, required, and urgent client medical appointments per applicable standards. The team supports clients who require intensive care including but not limited to toileting and bathing, special diets with personal and / or enteral feeding, and enhanced funding. This position is expected to participate in direct service as needed.
