HEDIS Outreach Nurse

Judge Healthcare is currently seeking HEDIS Outreach Nurses for one of our fantastic clients!

Location : Remote (Pacific Time Zone hours)

Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM PST

Employment Type : Temporary (12 / 01 / 2025 – 04 / 30 / 2026)

About the Role

We’re seeking a licensed Registered Nurse (RN) with HEDIS experience to join our Health Care Informatics team. In this temporary role, you’ll play a key part in improving 5 Star / HEDIS performance by engaging directly with members and providers to close gaps in care and ensure accurate documentation.

Responsibilities

Conduct outreach to members regarding clinical screenings aligned with HEDIS and 5 Star measures

Apply NCQA HEDIS Technical Specifications to assess compliance and guide member conversations

Accurately document member interactions and screening outcomes in the HEDIS database

Maintain confidentiality and integrity of protected health information

Identify missing documentation and follow up with members and medical offices via phone and fax

Escalate complex issues to the HEDIS Supervisor as needed

Collaborate with team members to support overall program goals and SCAN’s mission

Minimum Qualifications

High School Diploma or equivalent

Active California RN

1+ years of experience with HEDIS and Medicare measures

Strong understanding of NCQA HEDIS Technical Specifications

Excellent verbal and written communication skills

Detail-oriented with the ability to multitask and meet deadlines
