HealthPro Heritage is seeking a Part Time Speech-Language Pathologist (SLP) to join our therapy team in Durham, NC, within our Long Term Care division in a Skilled Nursing Facility (SNF) setting. This is a flexible, as-needed position ideal for therapists looking to make a meaningful impact with scheduling that fits their lifestyle.

Key Responsibilities :

Provide speech-language pathology services, including evaluation and treatment of communication, cognition, and swallowing disorders

Develop and implement individualized treatment plans to support resident goals and functional outcomes

Collaborate with an interdisciplinary team to ensure holistic, resident-centered care

Maintain accurate and timely documentation and communicate progress effectively

Responsibilities

• Patient Assessment : Conduct comprehensive evaluations of speech, language, voice, and swallowing abilities using standardized tests and clinical observations. Develop accurate diagnoses based on assessment results.
• Treatment and Education : Implement evidence-based therapies for speech, language, voice, and swallowing disorders; monitor and adjust treatment plans; educate and support patients and families on disorders, treatment strategies, and daily communication improvement.
• Documentation : Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards.
• Collaboration : Work with healthcare providers to ensure effective patient care and participate in multidisciplinary team meetings as needed.
