Start Date or Shifts Requested : ASAP - Ongoing

Schedule : Monday through Friday 8 am to 6 pm

Job Details

Home Health

In-home health and wellness assessments for health insurance plans. H&P evaluations. Will review medications / dosages, (will do physical, check blood pressure, finger stick, get urine sample, and will leave fecal kit for patient to mail in).

5 to 8 assessments per day (Assessments last approx.. 45-75 mins, last appointment scheduled @ 6 pm).

Ages seen : Geriatrics

EMR : Proprietary System. All documentation is done on the iPad.

Requirements : MUST HAVE CAQH, NPI #, NP license in state they are working, Unrestricted State license
