Case load will consist of up to 33 clients (often less)
Case managers will assess new admits, interact with families
Meet with each client at least once a week and assist clients with needs that could include; skills training, program work, aftercare planning, legal issues, amongst many other necessities.
Often connecting clients with outside resources such as disability, state services, etc.
Diligent note taking and documentation of sessions, communication with families and intercompany communication.
Attending staff meetings with therapists and providing / receiving updates on each client
Experience with case management preferred but not required
Working with medical team when necessary
On occasion transporting clients to appointments
Attend required trainings when needed
HS diploma / equivalent-Bachelor'sdegree preferred not required
