Job Description : Case Manager – Supporting Children and Families
As a case manager, you will play a critical role in the lives of children and their families by making timely, independent decisions that ensure access to essential medical, educational, emotional, and social services. As a dedicated case manager, you will coordinate vital resources and respond to the urgent needs of children and families. You will guide families through permanency planning, attend court hearings, and lead family meetings that promote stability and long-term success for every child you serve.
This position involves regular in-home visits with children and caregivers, strong collaboration with schools, courts, and community agencies, and a firm commitment to documentation, data tracking, and positive outcomes. As a case manager, your responsibilities also include transporting children to appointments, facilitating visitation, and helping secure permanent homes for those in out-of-home care.

Physical Demands : While performing the duties of this job, the employee is regularly required to communicate effectively. The physical requirements of this job includes : sitting, standing, walking and climbing stairs on a regular basis; lifting up to 25lbs, pulling and / or pushing on occasion.
Work Environment : The majority of this job is performed in the community. Will need to transport clients to activities or appointments
Other : Must be 21 years of age. Must possess a valid driver's license, reliable access to an automobile and valid automobile insurance is required.
