Physician, Associate Family Medicine

NorthPoint Health and Wellness Center is seeking a board-certified / eligible Family Physician to provide the full-spectrum of ambulatory family medicine in a community clinic setting. Our practice is a diverse urban community health center in North Minneapolis whose mission is partnering to create a healthier community. This is a part-time to full-time Family Physician position depending on clinic requirements and candidate preference.

The Family Physician will provide primary care medical services; perform diagnostic techniques; interpret diagnostic test results; solicit patient medical information; collaborate with appropriate personnel in planning patient care; participate in research activities; instruct ancillary and student personnel; and review records and reports. The candidate must demonstrate the ability to respect appreciate and promote teamwork among diverse co-workers and patients.

Location and hours :

This position will be on-site at NorthPoint Health and Wellness Center at 2220 Plymouth Avenue North Minneapolis MN 55411. This position will be part-time to full-time working between 30 - 40 hours per week. Clinic hours are Monday Wednesday and Friday from 8 a.m. - 5 p.m. Tuesday and Thursday from 8 a.m. - 7 p.m. and Saturday from 8 a.m. - 12 p.m. (work hours will vary).

In this position you will :

• Obtain health history perform physical examinations and conduct preventive screenings to provide the full-spectrum of primary care services for patients of all ages.
• Evaluate diagnose and treat acute and chronic illness and disease.
• Provide prenatal care services (non-hospital).
• Provide education and preventive services for our patients.
• Make appropriate referrals to other health professionals and community agencies.
• Participate in NorthPoints quality management program.
• Utilize the electronic health records Epic.
• Participate in the design implementation and evaluation of care programs based on the needs of our population.
• Serve as a resource to other staff and areas of the clinic.
• Guide and direct allied and ancillary personnel.
• Partner with patients and families to optimize their health and wellness.
• Provide leadership to further department-based strategies that align with county goals mission and core values for diversity equity and inclusion (DEI).
