Nikkei Manor Assisted Living, located in the heart of Seattle's International District, is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Wellness team. This role offers a meaningful balance of clinical care, ongoing assessment, and resident advocacy all within a supportive, home-like community where staff and residents truly know one another.

Overview
As a Wellness Nurse, you will play a key role in supporting the health and well-being of our residents. You'll use your clinical expertise to perform ongoing assessments, monitor health conditions, and ensure each resident receives the highest quality of care in a supportive, home-like environment.
Our nurses are part of a collaborative care team that includes Caregivers, Medication Technicians, and our Health Services Director all working together to promote dignity, safety, and wellness for our residents.
Responsibilities
Evaluate resident health status and follow through with appropriate treatment and or documentation.
Provide residents' medications and treatments that require administration by a licensed nurse.
Monitor and maintain records reflecting residents' physical and mental condition.
Coordinate and oversee medication and treatment services, ensuring accurate and complete daily documentation with no gaps in records.
Provide support and consultation to the Resident Assistants and Med Techs in problem-solving, service challenges, and resident health issues.
Act as a liaison between residents, doctors, and families regarding medical and social concerns.
Coordinate with residents and families on outside appointments and arrange KNW transportation if necessary.
Maintain current medication orders for residents.
Coordinate with Health Services Director for Assessments of potential residents prior to move-in, residents returning from hospital or skilled nursing facility for appropriate care and perform ongoing assessments to determine the need for additional or modified services.
Establish and maintain current knowledge of the requirements of regulatory agencies.
Respond to emergency situations and administer first aid as needed.
Investigate and document resident incidents.
Assist HSD to monitor resident incident reports and medication error reports to assess causes and preventative measures.
Assist Health Services Director with employee evaluations.
Attend mandatory in-services and training.
Handle all forms of resident Protected Health Information (PHI) in a confidential, private and secure manner.
Assist in instituting infectious illness outbreak-management with guidance from Wellness Director and Public Health.
Demonstrate familiarity with EHR systems (currently using PCC).
Arrange and coordinate Home Health, Hospice, and Respiratory Services.
Perform other duties as assigned.

Constant standing / walking; regular communication in person / phone; occasional lifting up to 25 lbs; assistance with resident transfers >
100 lbs as needed; bending, stooping, kneeling.
Primarily indoor, climate-controlled assisted living environment with low-moderate noise. Potential exposure to communicable diseases requiring PPE.
