Wellness Director Independent Living - 1st Shift

This role requires a person who is highly organized, able to coordinate care for a large independent living community, with the ability to communicate with residents, families and associates, to ensure resident appropriateness for independent living. This person should expect to be a liaison for residents for questions and concerns regarding health-related issues and provide resources for care. This position is a key member of the Leadership Team and will have responsibilities to include participating in At Risk meetings, Safety Committee meetings, assisting with vaccine clinics and any other duties as assigned.

Job Requirements

• Perform initial wellness interviews for independent living move-ins
• Case management activities, including but not limited to; coordinate with outside providers, maintain communication with residents outside the community for health reasons, provide medical resources and guidance to residents and families.
• Conduct resident care conferences
• Discharge planning and implementing care from hospitals / rehabs
• Review all incident reports and follow up as needed
• Ability to work in fast paced environment
• Highly organized
• Computer literacy a must
• Actively participate in Leadership Team
• Work closely with Sales Team on all move-ins
• Social Worker / Case Manager / LPN experience desired or equivalent
