Specialty Intake Coordinator

Responsibilities include :

• Coordinating infusion and nursing services for the pharmacy
• Handle incoming referral calls and completing referral forms
• Complete patient admit packets for all new patients and forward to the dispatcher
• Provide pharmacy with orders
• Communicate with and document all communications and authorizations with case managers, nurse agencies, patient / caregiver, physicians, insurance, and pharmacists into CPR+.
