Patient Aligned Care Team Social Worker
The Patient Aligned Care Team (PACT) Social Worker is assigned to outpatient clinics and provides consultation services to other staff within the clinic setting, division, and the medical center regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to treatment plan. Supports PACT members with challenging Veteran-care situations.
