Key Responsibilities of a Hospice RN / RN Case Manager :
Act as the primary contact for patients and families, guiding them through their hospice experience with empathy and expertise.
Deliver and document skilled nursing care according to each patient's individualized Plan of Care, whether in their home, assisted living, or wherever they reside.
Conduct initial and ongoing assessments to monitor patient conditions and adapt care as necessary.
Collaborate with an interdisciplinary team, including physicians, LPNs, CNAs, social workers, chaplains, and others, to create and continuously improve personalized care plans.
Provide emotional, physical, and spiritual support for both the patient and their loved ones, offering education, comfort, and guidance through bereavement.
Instruct caregivers on how to provide safe and confident care for their loved ones, combining practical training with emotional support.
Communicate significant updates regarding patient conditions to the care team, facilitating timely and appropriate interventions.
Maintain precise, up-to-date clinical documentation and adhere to infection control and compliance standards.
Support organizational quality improvement initiatives and scheduling efforts to ensure care remains compassionate, consistent, and efficient.
