What We're Looking For

Trillium Health Resources has a career opening for a High Fidelity Wraparound (HFW) Liaison to join our team! The High Fidelity Wraparound (HFW) Liaison is responsible for directly supporting, monitoring, and collaborating with state and other partners in HFW expansion efforts using a state existing fidelity monitoring system through the implementation of expansion funds. This position will be the point of contact for recruiting, identifying, developing, tracking, monitoring, and evaluating HFW in our region, as well as program outcomes and requirements, and developing and maintaining relationships with identified HFW providers, state partners, and community stakeholders. This position will work in collaboration with HFW partners to increase community awareness of HFW and expansion of Trillium's HFW provider network. This position requires some travel.
